Download the application form and mail the completed form to the AMHSI Executive Office at 78 Randall Avenue, Rockville Centre, NY 11570 with a check for the $350 registration fee.

Request for Official School Recommendation

Download the recommendation form and submit to your high school to complete and return to the AMHSI Executive Office.

When your application and recommendation is received... ...you will be contacted by a local Director of Admissions for an interview and upon acceptance, you will receive a follow up packet of forms. Upon acceptance to the AMHSI program, a deposit of $2,000 is required to reserve your place.
